Rewriting Rare Myasthenia Gravis Story Submission Authorization, Consent, and Release Form 2025

By submitting your story to Rewriting Rare ("Project"), you agree to the following terms:

1. Grant of Rights: You grant Rewriting Rare a non-exclusive, perpetual, worldwide, royalty-free license to publish and use your story in connection with the Project, including books, websites, and promotional materials. All proceeds, profits, and intellectual property rights from the Project belong solely to Rewriting Rare.

2. Editorial Process: Your story may be edited for clarity, grammar, length, or appropriateness in line with the Project's goals.

3. Privacy and Anonymity: Your submission should avoid violating privacy laws (e.g., HIPAA) and should not include specific healthcare provider or third-party details without explicit consent. Anonymity is encouraged but not guaranteed if identifying details are included.

4. Voluntary Submission: By submitting, you confirm the story is your original work, you have the rights to share it, and it does not infringe on third-party rights.

5. Indemnification: You agree to indemnify Rewriting Rare against claims arising from your submission.

6. Purpose: Your story will be used for informational and storytelling purposes, not as medical advice, or to be used as a replacement for the careful care and guidance of a licensed healthcare/medical professional.

7. Withdrawal of Consent: Submissions may be withdrawn by contacting Rewriting Rare, but withdrawal may not be possible if the story has already been published.

8. Data Collection and Usage
Rewriting Rare will collect and store your submission in accordance with applicable data protection laws. While every effort will be made to safeguard your data, you acknowledge that electronic communication is not completely secure, and Rewriting Rare cannot guarantee the absolute security of submissions.

9. Limitations of Liability
To the maximum extent permitted by applicable law, Rewriting Rare disclaims all liability for any direct, indirect, incidental, or consequential damages arising from your submission, including but not limited to breaches of privacy, unintended publication, or interpretation of the content.

10. Representation of Submissions
By submitting your story, you represent and warrant that:

• You are at least 18 years old or have obtained parental/guardian consent if under 18.

• The content of your story does not contain false, defamatory, or unlawful material.

11. No Guarantee of Publication
Submission does not guarantee that your story will be published or used in the Project. Rewriting Rare reserves the right to reject any submission at its sole discretion.

12. Use of Submitted Content
Rewriting Rare may use your submission for its intended purposes without obligation to notify you of specific uses. However, significant edits or adaptations for major publications will be made transparently and in alignment with the Project's mission.

13. Jurisdiction and Governing Law
This agreement will be governed by the laws of Florida. Any disputes arising out of or related to this agreement shall be resolved in Florida.
